Forms

To access any of the forms below scroll down to the form you wish to download. Click on small box in the upper right hand corner of the selected document to open. Use the download arrow to download your editable "Word Document" A sign-in to Google is not required to download these documents.  Then go to the download file on your computer and look for the "Word document" form you just selected.  (if the downloaded word document asks you to allow it to be edited please make that happen.)

The "Green Text" located in the documents below is there to assist the GAL by describing the data that is needed to replace the green text. If you would like to remove the GREEN color from the text just "highlight" the entire document (done the same way as copying) but instead go to your "Word" header and your will find a control dropdown for text color.  It is usually located in the same line as the "Bold" tab. Then select "Black."

THIS AREA FOR GOOGLE DOCS ONLY!!!  New 2/2024 JR Format

Our GAL Offices prefers you work in Microsoft Word.  However if you find yourself using Google Docs the information below may be helpful. You will probably be asked to sign in to your Google account so you can use Google Docs.  The completed documents will be kept in your Google Drive under Docs. Below are instructions for sending a Google Doc to your CAM in word. 

1) To use Google Docs without MS Word you must have "Google Chrome" installed in your device. (GAL Office computers usually have "Google search" but do not have "Google Chrome" installed.)

2) If you do not have Microsoft Word installed in your device you can do your JR in Google Docs. However when it is time to forward your JR to your CAM you must do the following.  

1. Click on 'File" 

2. Select "Email this file"

3. A dialogue box will appear with questions and selections.

4. Add your CAM or other person's email address

5. Leave a customized subject (Optional)

6. Add a message to CAM or other person (Optional)

7. Leave the don't attach box unchecked

8. The "File Type" box automatically shows "PDF" using the little dropdown arrow to the right select "Microsoft Word" and then click on "send".

3) To download in "MS Word" when you are in "Google Docs" you must click on "File"  then "Download" and then select "Microsoft Word"  If you have MS word installed in your device it will download as a Microsoft Word Document wherever you documents are set to download. ( ie. download file, desktop, etc.)

This Youtube video will show you how to send your  JR in Google Doc. format to your CAM in an editable "MS Word" format. Using the dropdown tab that Make sure you select the Microsoft Word dropdown
