Module 1: (March)
· Community panel at Orientation (next year replace with attendance at Presentation Day?)
· Review/refresh ILM content (post-test assignment, can be repeated until 100%)
Module 2 (June)
· Meet your PCHC teams and their community partners
· Learn about population or community and their identified needs for service
Mindful Wellness Day 6/26 with Dr. Hirsh and the Wellness team (tentative)
· Plan community level intervention (and/or activity for engagement mixer) in September
o Needs assessment – what MS3 skill gaps can faculty/community experts address
o Develop list of needed skill sessions (health literacy and creating pt ed materials, writing advocacy letters, motivational interviewing, social housing options, working w interpreters, SNAP resources, what is a FQHC, primary care psychiatry, med-legal partnership, funding search resources at the library)- over time these could be given by pathway students (pathway leaders considering now)
Module 3 (September)
· Conduct meaningful service toward community-identified needs
· Help partner prep for/participate in 9/24/25 mixer (mixer might include opportunity to attend skill sessions for attendees based on needs assessment in June)
· Reflective writing on how PCH perspective will impact pt care -for audience of community partners (and/or pathway project for pathway mentors)
Module 4 (March/April)
· Evaluate impact
· Prep for the next year’s learner activities
· Develop and give presentations
· Review peer presentations
· Meet with mentor re academic next steps (can include reading/responding to their peer reviews or reflections on how PCHC impacted their professional identity development)
