We understand that consent forms can contain a lot of information, and it can be easy to overlook certain details when signing. For that reason, we provide our full consent form here so you can review it at any time.
By signing our consent form and allowing Paws on State Pet Salon to provide services for your pet, you acknowledge that you have had the opportunity to read, understand, and agree to the policies and terms outlined below.
We encourage all clients to review these policies carefully and contact us with any questions or concerns.
All clients are required to sign our consent form either electronically or physically before their first appointment.
BASIC CONSENT FORM
Basic Consent
We strive to ensure your pet’s visit is as comfortable as possible. Please note, grooming may uncover underlying medical issues or exacerbate existing ones. By signing this, you acknowledge we prioritize your pet’s well-being and do not intend harm. We are not liable for pre-existing medical conditions unless directly caused by grooming as examined and determined by a licensed veterinarian. In case of emergency or inability to contact you, you authorize us to seek veterinary care from a provider of our choice.
Matted Pet Consent
By signing this form, you acknowledge that the groomer is authorized to act in the best interest of your pet. This includes shaving if necessary to ensure your pet’s comfort and safety in cases of severe matting. We exercise careful judgment to avoid discomfort or danger during dematting procedures. Matting can potentially conceal or lead to skin irritation, bruising, or injuries. You understand and accept that we are not responsible for any injuries or irritation arising from your pet’s matted condition. Tight matting on a dog’s ear can restrict blood flow to the tips, potentially resulting in a hematoma when shaved. We do not assume responsibility for hematomas arising from pre-existing matted conditions at the time of drop-off.
Vaccination (Rabies) and Illness
By signing this form, you confirm that your pet is currently up to date on its rabies vaccination administered by a licensed veterinarian. Proof of vaccination can be provided via paper, email, or a phone call to your vet. You acknowledge responsibility if your pet injures a groomer, understanding that their protection is ensured by this vaccination. While we recommend your pet to be up to date on its distemper vaccine, it is not mandatory. We adhere strictly to sanitary and cleaning procedures before, during, and after grooming each dog, and are not responsible for any illnesses.
Cancellation and Late Policy Fees
By signing this form, you acknowledge our policy regarding missed appointments or tardiness without prior notification. Our policy requires at least 24 hours notice for cancellations and no later than 15 minutes tardy to appointments. To maintain the smooth and safe operation of our services, it is essential that clients arrive promptly at their scheduled appointment time. You understand that habitual lateness or failure to attend scheduled appointments without notice may incur late fees and no-show fees. You agree to settle all fees before scheduling another appointment.
Price Quotes and Estimates
By signing this form, you acknowledge that all price quotes are estimates and may vary based on factors such as size, coat type, coat condition, breed, behavior, or any other unforeseen circumstances during the grooming process. Groomers reserve the right to adjust pricing for any service at their discretion, depending on the condition and behavior of the pet.
Owner Responsibility Disclaimer
While your pet is in your control, including but not limited to being on a leash, in your arms, or otherwise not in the care of our staff, we are not liable for any incidents that may occur. This includes, but is not limited to, dog fights, bites (to humans or other animals), or your pet slipping out of its leash or collar. You assume full responsibility for your pet’s behavior during this time, including any injury or damage caused to yourself, your pet, others, or property. Pet owners are liable for any injuries to a groomer if the injury results from behavioral issues that were known to the owner but not disclosed at the time of drop-off.
HANDSTRIP CONSENT FORM
Handstripping is a specialized grooming technique used to remove mature or dead outer coat by hand or with appropriate stripping tools. It is commonly performed on wire and harsh-coated breeds to help maintain the proper coat texture and appearance.
Handstripping is different from clipping or shaving and may require regular maintenance and multiple grooming sessions to achieve and maintain the desired coat.
By signing below, I understand and acknowledge that:
My pet is receiving a specialized handstripping service.
The results of handstripping can vary depending on my pet's breed, coat type, coat condition, previous grooming history, and individual coat growth.
If my pet has previously been clipped or shaved, the coat may have altered texture, color, or growth patterns.
A previously clipped or shaved coat may require multiple coat cycles before the proper texture can potentially be restored.
Occasionally, not all of the coat can be removed during one appointment.
Handstripping may require regular maintenance to maintain the desired coat.
The groomer will use their professional judgment regarding how much coat can safely and appropriately be removed.
I understand that my pet's comfort and safety are the priority during the grooming process.
I acknowledge that:
Some pets may be more sensitive to handstripping than others.
Temporary redness, sensitivity, or mild irritation may occur following the service.
If my pet becomes excessively stressed, painful, aggressive, or otherwise unable to safely tolerate the service, the groomer may stop or modify the service.
The groomer may determine that handstripping is not appropriate for my pet on a particular day.
I understand that every precaution will be taken to provide a safe and comfortable grooming experience, but no grooming service is completely without risk.
I understand that existing skin or coat conditions may affect the handstripping process and the final result.
I agree to inform the groomer of any known:
Skin conditions or sensitivities
Allergies
Previous reactions to grooming
Medications that may affect grooming
Areas of pain or sensitivity
Other conditions that may affect my pet's ability to safely receive this service
I understand that mats, tangles, packed coat, excessive undercoat, or poor coat condition may affect the ability to safely perform handstripping.
If my pet's coat is not in an appropriate condition for handstripping, I understand that the groomer may recommend an alternative grooming method or discontinue the handstripping service.
I understand that attempting to handstrip an unsuitable coat may cause unnecessary discomfort or damage to the skin or coat.
I understand that handstripping is a process and that the final appearance may not be exactly what I expect.
I acknowledge that:
The groomer cannot guarantee a specific cosmetic result.
Coat color, texture, density, and growth may vary from one pet to another.
Previous clipping or shaving may affect the final result.
My pet may require multiple handstripping appointments to achieve the desired coat.
The groomer may leave some coat in place when necessary for the health, safety, or comfort of my pet.
My pet's individual coat and condition will determine the amount of work that can reasonably be performed.
By signing this form, I confirm that I have read and understand the information above and have had the opportunity to ask questions regarding handstripping.
I voluntarily authorize Paws on State Pet Salon to perform handstripping services on my pet.
I understand the potential risks, limitations, and variables associated with this grooming service and agree to the professional judgment of the groomer regarding my pet's safety and comfort.
