Youth Applicaton: Find on-line and fill out the form on a computer and print it. Both the parent and the unit leader need to sign form. After this is complete, form is sent to Pacific Skyline (PACSKY) Council/Stanford District representative (Rand Mahoney)

Medical: Required for every scout and Adult Leader . There are 3 parts.

  • Part A: Informed Consent, Release Agreement, and Authorization

  • Part B1/B2: General Information/Health History

  • Part C: Pre-Participation Physical. Needs to completed by a medical professional. Only required for any event activity exceeding 72 hours (Summer Camp, extended backpacking trip, etc.)

Troop 52 Code of Conduct: Reviewed and completed annually by both scout and parent/guardian.
