Please fill out the form completely and accurately. Make sure to provide all necessary information and double-check for any errors.

Step 2: Pay the necessary membership fees.

New Members:

600 pesos - one-time membership fee

600 pesos - annual dues for 2025 (per calendar year)

200 pesos - permanent ID fee

Renewing Members:

600 pesos - annual dues for 2025 (per calendar year)

Inactive Members (non-payment of annual dues, two years or more)

600 pesos - one-time membership fee

600 pesos - annual dues for 2025 (per calendar year)

200 pesos - permanent ID fee

Step 4: Attend the PASWI Membership Orientation to know the accomplished and future projects, programs and activities as well as international linkages of PASWI. (Schedule to be announced)

The COGS is a requirement for Registered Social Workers renewing their licenses under Republic Act No. 10847 and the PASWI is the current AIPO per PRBSW Resolution No. 07 series of 2024.

To ensure timely processing, please submit your COGS request at least 30 days before your PRC renewal appointment. This allows the chapter sufficient time to prepare your document and helps you avoid any inconvenience.

A certification fee of ₱150 will be collected for the preparation of your COGS. This fee contributes to the chapter's funds for projects, programs, and activities.
