If you have not had an appointment with me in the last 12 months you will need to fill it out again. Alot can happen in a year, doing this will help update your hair goals and re-evaluate anything that might have changed since your last appointment.

No problem! Any big or drastic changes, just fill out what applies to the changes in the form so I can ensure I have what I need to carry out your service.

I don't like or want to have to charge cancellation fees. I do all of my booking online with a nonrefundable deposit. The deposit goes towards your full services amount and it just helps break up the payment for you and protects my time.
