1) Complete an online application, do NOT pay any fee (just say pay by check I think there is that option), but must finish with a submit and ignore the invoice.

• ID: Front of driver license (cell phone picture is fine)
• CORI filled - that is NOT the same as the last page of application. CORI is for MA
• Youth training certification into this folder. Make sure the file name is Your first and last name - it should already by default emails to you.
