Any condition or disease that is severe that has not responded to other treatments, and that can “reasonably can be expected to be relieved” with medical marijuana can qualify a patient for a medical marijuana license. Conditions that may qualify a patient for a medical marijuana card include but are not limited to:
Anxiety
Anorexia/poor appetite
Chronic pain
Severe nausea
Severe insomnia
Seizures
Severe or persistent muscle spasms
Glaucoma
Post-traumatic stress disorder
Schedule a medical visit for evaluation and completion of a Physician Recommendation form (PRF). After a comprehensive review of your medical history your doctor will determine if you qualify for a medical marijuana license (card) for the state of Oklahoma. Your visit can be performed in person or via telemedicine if you prefer. If you qualify you will be given a signed copy of your PRF. Medical Marijuana examinations are FREE for OUTLAW MEDICAL members or $125 for nonmembers.
Once you have a completed Physician Recommendation Form (PRF) you will need to apply for your license/card with the Oklahoma Medical Marijuana Authority (OMMA). Registration requires proof of identity (drivers license, state ID, passport, or tribal card), proof of residency (drivers license, utility bill), a passport style photo, and the signed PRF (must be signed within 30 days of application). To apply for your medical marijuana license (card) go to OMMA's website here. Oklahoma does have an application fee of $100. The fee is only $20 for Medicaid, Medicare, or 100% disabled veteran patients.
*If you are "technologically challenged" we will be happy to help you through the process of applying during your visit.
After applying for your license you should receive a determination of approval or denial in 14 business days, then your card will be mailed to you. Patients are able to visit licensed Oklahoma dispensaries to purchase medical cannabis. If you aren't sure where to go we can help you find a dispensary in your area.
