After your My Study Abroad application has been accepted by Gies and you have committed to the program, you can start filling out the Otago application. You do not need to wait for an email from Otago to begin your application. Go here for detailed instructions and the link to the application.
A requirement of New Zealand's Tertiary Education Commission is that Otago must have a certified copy of each student's passport on file prior to them beginning studies. You can obtain a certified copy here: https://studentlegal.illinois.edu/international/document-certification. After Otago reviews your application, they will email you further instructions regarding courses, housing, and fees.
