FUNDING REQUEST FORM - This form is used to request funds for special clubs, activities, assemblies, etc.
TEACHER REIMBURSEMENT FORM - Full time OSW staff are given up to $100 towards materials that they have purchased for their classroom. Part-time OSW staff are given up to $35 dollars. Please be sure to submit all receipts with this form.
CHECK REQUEST FORM - This form is used to request reimbursement for materials that were previously approved by the OSO.
