Cancellation Fee: Failure to attend your scheduled therapy appointment without notification to reschedule or cancel will result in a charge reflecting a fee of $60
Failure to cancel or reschedule with less than 24 hours notice will result in a $50 inconvenience fee.
To avoid all fees, please get in touch with us to cancel and or reschedule 24 hours before the scheduled appointment.
Orchard Road Counseling Fee Disclosure

While filing insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date of which services are rendered. It is your responsibility to know any stipulations of your insurance such as copay amount, need for referral forms, deductibles, and need for treatment pre authorizations. This includes keeping up with the number of sessions allowed by your insurance coverage.
Please bring any paperwork that requires my attention to the session, so we can address it at the beginning of the session. If we are unable to finish during session time, I do have a fee for paperwork additions post session.
Copays are due at the beginning of each session
Your appointment time is reserved for you. I do not double book sessions. If you cancel at the last minute or no-show, there is a fee already covered in this paperwork but we wanted to mention it again. Insurance will not cover this fee and you will be responsible for the $140 fee.
If you are private pay, out of pocket. The intake session is $140 - this is a one hour session. Further sessions are also $140 for a 50 minute session.
Other fees:
For unpaid charges after 30 days, a 2% late fee will be added to your account. If past 60 days, your services may be interrupted.
$25 fee for any paperwork completed outside of session
$100 for a written report for any purpose (this fee must be paid before it is released to you)
$25 for any phone call that lasts more than 15 minutes
$140 for any consultation or review of records
$200 per hour for any time out of the office provided for court testimony
Record request fees can be found in the informed consent paperwork, as set out by Alabama Law
ALL unpaid bills will be turned over after 90 days to District Court Small Claims following documented attempts to satisfy charges owed. This is in accordance of Alabama Code of Ethics, Section A.10.c
