Intake information: 

Please make sure you bring your copay, as well as your insurance card to first intake session if you are filing insurance. Remember you are responsible for all payments, including verification of insurance coverage. Also, you will notice our office is located inside Lighthouse Church. We are not a part of their church organization, but we do have a space here! Dr. Amberson will meet you at the door and greet you to show you to the office. There are a few doors on campus, so please look for the flag pole and you'll see our sign on the doors closest to that location. The doors are located under a large awning.
