We have provided a template for an optional “Survey on Organizational Grievance Reporting Procedures”. The survey is meant to foster reflections on the organization’s current policies and procedures and prepare participants for the group discussion.
The survey can be used to set the stage for a reflection discussion meeting.
Prior to sending the survey out, the facilitator should complete the template, adding their contact information.
The facilitator should identify the best way to distribute the survey, including by email, hard copies, or transferring the questions to an online form.
Confirm confidentiality procedures - Whether or not the surveys are collected by the organization, or solely used by participants as a personal reflection tool should be determined. The confidentiality of any data collected should be confirmed by the facilitator with organizational leads, and clearly communicated to participants.
To use the survey, make a copy of the Google doc, or download the doc to customize it.
