Important! After you fill out the form, you must print it out and send it to us. The form itself tells you how to do this. Note that this is the case even if you fill out the form online. In some cases, you must get the form notarized when you sign it.

You can give permission to a person to contact Service Canada on your behalf by signing in to your My Service Canada Account or by completing the Consent to Communicate Information to an Authorized Person form (ISP-1603).

Note: You could be fined if there is false, misleading, or omitted information on your CPP retirement pension application. Interest is also charged on penalties and overpayments that are subject to a penalty.

The best way to file for Veteran Readiness and Employment services is to apply online at va.gov/careers-employment. To submit a paper application, download and complete a VA Form 28-1900, "Disabled Veterans Application for Vocational Rehabilitation" and mail it to your local regional office. You may also go to your local regional office and turn in your application for processing.

Download and complete VA Form 26-1817, "Request for Determination of Loan Guaranty Eligibility - Unmarried Surviving Spouses. You can also call 1-877-827-3702 and follow the prompts for "Eligibility" and we will send the form to you. 


Mail your completed application to the Regional Loan Center that serves your state.

PBGC may ask workers and retirees to provide additional required information, as appropriate. Some of these forms may be submitted electronically via My Pension Benefits Access (MyPBA), an online service for participants to complete transactions with PBGC. Participants in PBGC-trusteed plans can use PBGC's fast, free, and secure online service tool to apply for pension benefits, update contact information, adjust federal income tax withholding, and more.

If you wish to register to vote, please download and complete the Voter Registration Form and return it to your local FSD office. This has no impact on your application or continued eligibility for assistance.

These applications are only available to customers who qualify for retirement. When you are within one year of retiring, you must request an official benefit estimate from DRS before online retirement or a paper application is available to you. The fastest way to send in your documents is through our secure Message Center. Registered users can log in to upload forms and attach all required documentation. If your form requires a signature, you must print, sign, and then upload the signed page.

If you become aware of the death of any person receiving pension benefits from the Pension Fund, please notify the Pension Fund immediately by clicking here to complete the online form or by filling out the Death Notification Form and returning it to us.

We have provided pre-approved sample model language which can be found below. The language differs depending on whether or not you are currently receiving Retirement Benefits. Please choose the form that applies to your divorce or separation circumstance.

IF YOU ARE MARRIED (CHOOSE ONLY ONE OF THESE THREE OPTIONS):

Joint and 50% Surviving Spouse Option: If you are MARRIED when you retire, the normal form of payment is the Joint and 50% Surviving Spouse Option. The Joint and 50% Surviving Spouse Option provides for a reduced monthly payment for your lifetime so that in the event you die before your spouse, 50% of your reduced monthly benefit willcontinue to your spouse for the remainder of his or her lifetime. This option is revocable by the member up until 90 days of receiving a pension benefit, and cannot be later revoked or changed under any circumstances. You must include copies of your marriage certificate and your spouse's birth certificate for this option.

Joint and 75% Surviving Spouse Option: If you are MARRIED when you retire, you can elect the Joint and 75% Surviving Spouse Option. The Joint and 75% Surviving Spouse Option provides for a reduced monthly payment for your lifetime so that in the event you die before your spouse, 75% of your reduced monthly benefit will continue to your spouse for the remainder of his or her lifetime. This option is revocable by the member up until 90 days of receiving a pension benefit, and cannot be later revoked or changed under any circumstances. The option is not available for plan members whose death occurs before their retirement date. You must include copies of your marriage certificate and your spouse's birth certificate for this option.

The first step toward receiving your pension retirement benefit is to fill out the application and either print and mail it back or use the Message Center. Everything you need to get started is included in the application, and you must fully complete each section to avoid a delay. We recommend that you send us the application at least 90 days (but not more than 180 days) before your retirement date.

Once you've obtained a benefit estimate and have decided on your payout option and retirement date, complete a retirement application in your Online Pension Account or submit a retirement application form on or shortly before your last working day. You may also wait until after your last working day, but most members feel better having completed their application before leaving employment.

In 2012, the State Legislature passed a law returning the authority to grant pension cost of living adjustments (COLAs) to the Legislature and also requiring 100% funding of the plan before a COLA could be awarded from WRS funds. The Public Employee Pension Plan is moving in the direction of 100% funding through investment earnings and contribution increases. However, it will be many years before the plan meets the requirements to grant a COLA from WRS funds.

Benefits provided under the pension plans administered by the Wyoming Retirement System represent one aspect of your retirement assets and should not be expected to replace one hundred percent (100%) of your pre-retirement income. Cost-of-living and other benefit increases are not incorporated into your benefit, emphasizing the importance for you to build additional resources for retirement income, such as personal savings. However, members who select the self-funded COLA feature as a benefit option will see a guaranteed increase.

Direct Deposit of your monthly benefit payment is now required for all applications received after January 1, 2010. The required form is included with the retirement application below. If you are currently on direct deposit as an active employee, we can use the same account you are currently using. Please complete your retirement application on your computer before printing it out and mailing to us.

Please choose ONE "New Member" form, either the "Commonwealth" OR "NON-Commonwealth" version. Also, please ask employees if they have prior service that can be transferred so that their contribution rate is calculated accurately.
