Diabetes Care Policy

In relation to 504 plans and in accordance with Ohio Revised Code 3313.712 and 3313.713, no later than fourteen days after receipt of an order signed by the treating practitioner of a student with diabetes, the OHVA 504 Coordinator shall inform the student's parent, guardian, or other person having care or charge of the student that the student may be entitled to a 504 plan regarding the student's diabetes. The 504 Coordinator shall use the Information Letter to Parent and the Parent Permission Form for 504 Evaluation recommended by the Ohio Department of Education.
