In accordance with federal and state guidelines for identifying students with special needs and providing all students with educational support, it is recommended that parents provide the school with all documentation, including any medical history that identifies a diagnosis. Although a specific diagnosis is not actually necessary if the school determines a student is substantially limited in a major life activity and that limitation is caused by a mental or physical impairment, the parent/legal guardian is responsible for providing the information to school officials. It is critical that the information is submitted so that attendance and/or academic progress is not impacted due to the student’s inability to complete the course requirements due to a medical, physical, or mental condition that has a negative impact on learning and progress. If the student has a medical, physical, or mental condition that is negatively impacting their education, parents can submit documentation of a diagnosis from a qualified licensed physician to the school. The notification and documentation must be submitted as soon as possible after a diagnosis has been made or within ten days of the school requesting documentation in order for students to avoid lack of progress in their courses and any truancy issues. 

The 504 Plans are monitored and maintained by the 504 Coordinator, and are kept in a secure location available to the students' teachers.
