Waiver

All participants in OCD Volleyball Events (league or tournament) will be required to sign a Release of Liability Agreement prior to playing. Unlike other organizations, OCD Volleyball pays directly for insurance rather than passing the costs on to its players.  By signing this document electronically, you agree your electronic signature is the legal equivalent of your manual signature on this Agreement and you consent to be legally bound by this Agreement's terms and conditions. 

Starting in 2021, this waiver will only need to be filled out once and is good for all future OCD Events if you are above 18 years old.

Why you may want to fill out the waiver again after 2021:

• Your information changed (name, address, etc.)

• You want to change your emergency contact

• You filled out the waiver as a minor and are now 18

Please follow the instructions in the section below for the waiver that applies to you. 

Juniors will be allowed to participate in all of our events EXCEPT for Players Ball.

Follow these directions. 

**No paper form will be available at the event site. Verbal permission or permission other than this legally binding waiver will not be accepted. No exceptions will be made.
