You will have to request a DOD safe drop from your recruiter prior to being able to send forms
1) ASTB / OAR Scheduling Request Form: Please fill out completely, then email this back to your recruiter with 3 potential dates that you will be available for testing. We only have one testing computer, and do not want to schedule more than 1 tester for any time slot. Available times are 0830 or 1200; Monday, Tuesday, Wednesday, and Thursday.

Bring the following with you to your test:
1) Birth Certificate
2) Social Security Card
3) Drivers License
4) Passport
A reference for you application is very important as it adds credibility to your application.
Ensure you obtain at least one employer reference if not more. Character references can be people such as College/ High School teachers, Coaches, Fellow Employees or anyone that can speak about your background professionally.
Please fill this form out and provide it to your recruiter. It will be sent out to all the references that you list. In the case that a form is not received by your reference you can email the form listed below to them to fill out. Please ensure if you do this that it is emailed or mailed directly from your reference back to your recruiter.
Request for Reference
This is the reference form that will be sent to your Instructor/Employer/Peer's you have indicated. These should be the same references that you list on your background check (SF86/NASIS).

MEPS is where you will go to complete a full doctors physical. In order to schedule a physical the forms listed below need to be completed.
The APSR gives the Navy a snapshot of who you are. Much like a resume or CV.
Application Processing and Summary Record:
Fill out pages 1 through 3 to the best of your ability.
Page 3 Motivational Statement: Explain why you want to be a Naval Officer. You can do this on the form or in Microsoft word and send it to your recruiter.
Motivational Statement Guidance.
Focus motivational statement on articulating what you can bring to the NAVY.
Discuss why you want to be an active duty Navy Officer.
List a quantifiable record of leadership management of supervisory experience as well as how you worked well in a team and as a leader.
Either have the transcript mailed to our office or emailed directly to your recruiter. Please do not pick up a transcript directly from your college.
If you are still enrolled and attending college please also complete the items below to show that you are currently attending.
