General Application Information
This year all applications will be online!
Important Items to Note:
When completing the application you will need to know who is responsible for payment, for example, Elks Lodge, School, Municipal Alliance, or Self.
Students and Parents should complete the application together as the parent signature is required.
Medical Information is required on the form as well as insurance information.
If a student inputs the Advisor email address the Advisor will receive a portion of the application.
You will receive an email confirmation that your application has been received!
Path to Leadership Conference Application Information
The cost of the conference is $50 per attendee which includes a welcome snack upon arrival, all program materials, lunch, conference backpack and more.
Teachers, Parents and guardians are welcome and encouraged to attend.
The deadline to register is January 10, 2024.
Peer Leadership Conference Application Information
The cost of the conference is $385 per attendee.
Advisors are strongly encouraged.
The deadline to register is December 16, 2023.
Payment Information
Path to Leadership Conference and Peer Leadership Conference payments should be made separately. Please do not combine payment on one check.
Indicate on an attached paper or in the memo of the the check the individuals the check is paying for, as well if it is for the Path to Leadership Conference or Peer Leadership Conference.
Checks should be made payable to NJ State Elks Association (NJSEA).
