Basic Passport Checklist

Step 1 – Pick Form 

• 📄 DS‑82 — For eligible adult renewals (previous passport meets criteria) 

• Current passport is undamaged and can be submitted with application  

• Issued when you were age 16 or older  

• Issued within the last 15 years  

• Issued in your current name (or you can provide legal name change document) 

Step 2 – Gather Documents 

• 🛂 Most recent U.S. Passport  

• 📸 1 Passport photo (2×2", white background) -if providing your own 

Step 3 – Prepare Fees 

• 💵 Passport Book: $130 

• 💳 Passport Card: $30 

• 📦 Both: $160 

• ⚡ Optional Expedited: $60 

• 📬 Optional Overnight return: $22.05 

Step 4 – Submit 

• 📮 Mail renewal to the address listed on DS‑82 instructions 

• Include payment and any required additional documentation 

⏱ Processing Times 

• 🕒 Routine: 4–6 weeks (+ mailing) 

• ⚡ Expedited: 2–3 weeks (+ mailing) – $60 

• ✈️ Urgent Travel: Appointment at passport agency (proof required) 

• 🆘 Life-or-Death Emergency: Same/next-day appointment (proof required)

• 🆔 Photo IDs for both parents/guardians 

• 📸 Passport photo (2×2", white background)-if providing your own 

• ✍️ DS‑3053 – Notarized with Photocopy of ID (if one parent absent) 

• Or proof of sole legal custody 

• ✍️ DS‑5525 – Special circumstances (if applicable) 

• ✍️ DS‑64 – Lost/stolen passport (if prior passport lost/stolen) 

Step 3 – Prepare Fees 

• 💵 Passport Book: Adults $130 / Children $100 

• 💳 Passport Card: Adults $30 / Children $15 

• 📦 Both (Passport Book & Card): Adults $160 / Children $115 

• 🏢 Acceptance Fee (DS‑11): $35 

• ⚡ Optional Expedited: $60 

• 📬 Optional Overnight return: $22.05 

Step 4 – Submit

• 👫 Children: Both parents/guardians must be present (unless proper forms provided)
