Create a Finals Forms account by November 1st  (link below)

Have a ParentSquare app downloaded

Pay Transportation Fee (link below) or check made out to Northridge High School (CANNOT be combined with suit money)

Print out Physical Forms (link below), fill out Physical History, Consent, Acknowledgement, and Release then take to doctor's appointment for them to fill out the Physical Examination portion.

Upload all Forms into Final Forms (athletes CANNOT practice until they are in Final Forms), keep paper copy for your records.

Pay for suits by check ($55 for girls and $38 for boys) made out to Northridge High School

Away and Sectional Dinner Contribution - $25 cash or check to Marissa Gardner

Raider swim caps must be worn during competition, if your athlete does not already have one, latex is $10 and silicone is $17. Check to be made out to Northridge High School or cash

PARENTS OF SENIORS ONLY - Jeff Douglas will be at team photos to take banner photos. This will be $75 each and banners will be $40. Marissa will keep track of the money and pay for these all together.
