Health Documents

For the 2025 Season, all wrestlers will need to submit both the Physical Form and Health History Update Questionnaire if your physical was after July 1st of this year.  If you have already had a physical before July first, the old paperwork is still accepted.

THERE ARE ONLY TWO DOCUMENTS THAT NEED TO BE SENT TO THE SCHOOL NURSE BEFORE YOUR CHILD CAN PRACTICE

#2 HEALTH INFORMATION

IF YOUR CHILD HAS NOT HAD A PHYSICAL IN 12 MONTHS, YOU WILL NEED TO GET ONE. USE THE PHYSICAL FORM. 
IF YOUR CHILD HAS HAD A PHYSICAL, FILL OUT THE HEALTH HISTORY QUESTIONNAIRE.

  This entire packet needs to be printed out.  The first 4 pages need to be filled out by your Medical Provider which they will keep.  Page 5, the last page, will be filled out and given back to you, so you can hand in to your child's school nurse. See screen shot below
