There is also a really interesting transition project called 5-8 Continuity Project for primary and secondary teachers to work together to facilitate a coherent transition between KS2 and KS3. This is aimed at teachers and subject leaders to help develop and it may be particularly suitable for linked ‘families’ of schools: primary, secondary.
