Monthly Report Forms

Each month, clubs are expected to submit a Monthly Report Form, or MRF, to the district. These forms include important information like service hours completed and the amount your club has fundraised to go toward your projects. 

These forms are for the first day of the month to the last day of the month. Forms are due on the 5th of the following month. For example, all service and fundraising from May 1 to May 31 must be reported in the MRF due on June 5. MRF's are typically completed by the club secretary, though each club can determine the best way for them to complete these.
