Complete, print an Annual Health and Medical Form (parts A&B only, plus a copy of insurance card both sides) for each family member that will attend pack or den events. These should be turned in to the Pack Committee Chair. TIP: Fill this out electronically and save the file to edit later as needed. You will use and print this file many times during your scouting career!
