MEDICAL CHECK FOR TATTOOS AND PERMANENT MAKE-UP:

Please aid Nadia in ensuring the best possible tattoo for you by answering the following

1 Are you over 18? If you are between 16 and 17 your legal guardian needs to be present at your tattoo appointment to sign a consent for on your behalf.

2. Do you have any allergies or skin disorders? Eczema/iodine

3. Are you on medication? High blood pressure/antibiotics

4. Do you have any medical conditions? Diabetes

5. Ladies: Are you pregnant or breastfeeding? FYI: Take in consideration that tattoos are more painful during your cycle.

BANKING DETAILS:

Please pay a deposit of R200, no deposit, no booking. The deposit is refunded on the day of the booking and lost if the booking is cancelled less than 48 hours prior to the booking, Once I’m notified by the bank of your deposit I will book a suitable time for you and start on the design
