Background checks are required to volunteer at any school in the South Bend Community School Corporation. Complete yours using one of the online forms below and it will be good for one academic year.

After submitting the online background check Google form, you will receive an email with a copy of your responses right away. This can be used as confirmation that you completed the online form.

You will receive an email notifying you once your background check has been approved. The school(s) you listed on the form will also be notified once your background check has cleared.
