Disaster Evacuation Plan: If an emergency situation results in the evacuation of the school premises, we will be relocated at the Henry County Sheriff Office. Custody Disputes: Parent(s) must provide a copy of any custody/care agreement/arrangement or other court-issued document regardless of any verbal agreement a parent/family may enter. Do not ask staff members to make exceptions. Custody disputed must never interfere with the ability of Mrs. Davis' After School Academy to provide for the health, safety, and comfort of the children in their care.
Dismissal from Program: Not all children are successful in a group setting. Mrs. Davis' After School Academy, LLC provides care for groups of children at the conclusion of the school day. If a child is difficult to supervise, disrespectful of the staff, intetionally destroys property or takes action that causes the health or safety of other children to be at risk, the child's parent/guardian will be called to pick their child up immediately. If the parent is called on two addition (three total) occasion, the parent/guardian will receive written notification suspending any agreement to provide child care services for the child.
Food from Outside Source: Mrs. Davis' After School Academy provides a nutritious snack after school. Our menu will be posted on the parent board. Please note if you would like to provide snacks or treats, enough will have to be provided for everyone. Food must come in a commercial vendor (i.e., these vendors include unopened packages from the store.)
Daily Routine: Mrs. Davis' After School Academy is an enrichment program that begins at the conclusion of each school day and continue until 6 pm Monday-Friday. A Be- weekly schedule will be posted on the parent board.
Reporting Abuse and Neglect: Child care staff are mandated to report all cases of suspected abuse or neglect of a child to a local law enforcement agency or the Department of Kentucky State Police; the cabinet or its designated representative; the Commonwealth's attorney or the county attorney. To report suspected abuse and neglect, call DCBS at 1 (877) 597-2331. In case of an emergency, dial 911 first. (2) An incident of child abuse or neglect shall be reported to the cabinet pursuant to KRS 620.030.
Medication Administration: Mrs. Davis' After School Academy will administer medications for chronic/ongoing illness or medications that are considered life-saving medication; i.e., resue or emergency medication.
Medication for Chronic or Ongoing Conditions: If your child must receive medication between 3pm-6pm daily, a release form must be completed for the 2020-21 school year (Blanket Release Form is included under the Apply tab). The release form and the medication must be provided to the staff on duty at the After School Program prior to the first day of attendance.
Rescue or Emergency Medication: If your child has an emergency medication or reuse medication for a severe medical condition, please assure the medication is provided to the After School Academy Staff and have your physician complete the required paperwork prior to your child's first day of attendance. A copy of our Emergency Medication/Rescue Medication release form to be completed by the physician is included in this packet.
Sick or Ill Children: Parents will be notified immediately if their child is not feeling week enough to participate in activities. Any time a child exhibits symptoms of illness, the staff at Mrs. Davis' After School Academy will notify the parents so arrangements can be made for the child to be picked up from the after-school care. Symptoms of illness include but are not limited to: complaining of pain or discomfort, fever over 100 degrees, diarrhea, vomiting, sudden onset of rash.
Section 7 First Aid and Medicine
(2) A child showing signs of an illness or condition that may be communicable shall not be admitted to the regular child-care program.
(3) If a child becomes ill while at the child-care center:
(a) The child shall be placed in a supervised area isolated from the rest of the children;
(b) The parent shall be contacted immediately; and
(c) Arrangements shall be made to remove the child from the child-care center as soon as practicable.

Children and Parent's Rights
Pursuant to KRS 199.898 Rights for children in child-care programs and their parents, custodians, or guardians -- Posting and distribution requirements.
(1) All children receiving child-care services in a day-care center licensed pursuant to KRS 199.896, or from a provider or program receiving public funds shall have the following rights:
(a) The right to be free from physical or mental abuse;
(b) The right not to be subjected to abusive language or abusive punishment; and
(c) The right to be in the care of adults who shall meet their health, safety, and developmental needs.
(2) Parents, custodians, or guardians of children specified in subsection (1) of this section shall have the following rights:
(a) The right to have access to their children at all times the child is in care and access to the provider caring for their children during normal hours of provider operation and whenever the children are in the care of the provider;
(b) The right to be provided with information about child-care regulatory standards, if applicable; where to direct questions about regulatory standards; and how to file a complaint;
(c) The right to file a complaint against a child-care provider without any retribution against the parent, custodian, guardian, or child;
(d) The right to obtain information from the cabinet regarding any type of licensure denial, suspension, or revocation of an operator, and cabinet reports that have found abuse or neglect by an child-care provider or any employee of a child care provider. Identifying information regarding children and their families shall remain confidential;
(e) The right to obtain information from the cabinet regarding the inspections and plans of correction of the day-care center, the family child-care home, or the provider or program receiving public funds within the past year; and
(f) The right to review and discuss with the provider any state reports and deficiencies revealed by such reports.
(3) The child-care provider who is licensed pursuant to KRS 199.896 or certified pursuant to KRS 199.8982 shall post these rights in a prominent place and shall provide a copy of these rights to the parent custodian, or guardian of the child at the time of the child's enrollment in the program.

Attendance
Site Directors will record the time each child arrives at the program daily and will initial the entry.
The parent/authorized pick-up person must record the departure time and initial daily.
At the conclusion of each week, the parent/authorized pick-up person must review the record and sign their name verifying that the information is correct. NEVER sign off on this ahead of time -- review the entries and sign the form on the last day your child attends each week, or the first of the next week.
If there is a change in your child's schedule and they will not be in attendance, please email, text, or call the site director's cell phone (502) 220-2905 prior to 3:00 pm.
Only persons whose name appears on the authorized pick-up list, may pick your child up from care. Changes must be made in writing.
People who are picking up a child from care may be asked to present a photo ID to verify identity. We apologize for this inconvenience -- we just want to keep your child safe.
