Please Fill out the form and email it to residency@ohlone.edu. Allow 10 business days for processing. Be sure to include a copies of your residency documents based on status. 

IMPORTANT: You will not receive your Ohlone ID# without filling out the form and including a copy of relevant documents. 

• a Valid Photo ID such as School Lunch Card, Drivers License, Passport, etc.  and 2 Proofs of Residency is Required.

If you're undocumented you still have to fill out the form. If you don't have any of the requested documents that is okay. We will work with you! No information is being share with outside resources
