• Permission Forms are released every semester a high school students wants to take an Early College course.

Utilize youtube tutorials to get started with the online permission form! 

Once you login and create a dynamics account you should have acess to the form where the student will fill out there information such as there name, DOB, and most importantly there CCCID# AND CCC confrimation number which they should have got from there CCCApply Application. If you have not applied for Ohlone for the semester please go back to the application page to get started. 

Following inputing the students information the student will also need to fill out there high schools information such as the name of there high school and the principles infomation please be sure his email is correctly spelled. 

Later the student will be asked to fill out a parents name and email so they can also sign the form.

After (you) the student submits you'll need to help guide your parent/guardian sign on there behalf through there listed email. 

They will create an dynmaics account on there own, read the terms, and apply! 

Following there submission - Dr. Fisher will then reiceve a request to sign. 

After all signatures are complete, Ohlone College will process students in 7-10 business days 

Follow up with me once your permission form is processed. Student will recieve a welcome email from a no-reply email address signaling there application is process with next steps and there Ohlone ID#. Student will need to complete self-placement survey, and clear and pre-reqs before registration day!
