Here is an example of a Dual Enrollment Permission form. You'll have to pick up a physical copy from Mrs. Felt in the Career Center after applying on opencccapply.

Some courses require a pre-req form. Please see an example below if necessary.

Your personal information is probably the most important step on this form. Please fill this out in BLACK ink and have your OpenCCCapply ID and confrimation number.

Parents/Guardians need to sign the form in BLACK ink and read the FERPA code.
