Do you want to do any repair works or any maintenance site activities at all our MRT Park & Ride Kajang and Putrajaya Line? If yes, you just need to follow all the following steps by fill in the Permit To Work (PTW) application form.

Note: 

Permit To Work (PTW) application needs to be applied for EACH site and applicable for ONE work duration only. You are NOT ALLOWED to apply for the work permit for the whole one year. Each time you need to conduct the work, you need to apply for the work permit. For instance, if you want to perform the work every week, you need to apply for that work permit on a weekly basis. 

Information:

1.  Firstly, please download the work permit application form and fill in all the required needed.

2.  Secondly, please fill in all the required needed to the google form completely.

3. If done fill in the work permit application form, then upload it inside the google form.

4. Please ensure to attach together all the supporting documents such as method   statement, work plan etc for our record and approval.

5.  Therefore, we will email to you back once the PTW application is approved. If rejected, you must submit back the PTW application. 

6.  Upon approval, please print out the work permit and together with the supporting documents (via Hardcopy) to be handed over to the car park operator before work  begins.

7.  Notes: Please sign the PTW form in the column E after work done together with the Parking Operator to endorse if the work at site from the vendor is completed.  

8.  Lastly, Please be ensure that you are required to comply with all safety regulations and maintain cleanliness at sites all the times.
