COVID DISCHARGE CHECKLIST
Run a brief social determinants of health screen on ALL patients at least once before discharge: lean on your colleagues in social work, case management, ACT, and pharmacy to clarify resources available for specific needs.
For patients on MassHealth, fill essential meds for 90 days (now covered if requested by patient or MD). Does not include meds that require PDMP reporting, behavioral health medications, antibiotics, or IV meds.
Place an ambulatory Social Determinants of Health referral for patients who have SDH needs and an MGH PCP - social workers, resource specialists, and navigators at MGH clinic sites are staying up-to-date on COVID-specific resources, which change everyday.
Include the dot phrase .HMUCOVIDDCINSTRUCTIONS in your AVS summary (includes multilingual text and phone options for patients to get COVID info).
Print language concordant COVID discharge instructions to include with discharge paperwork (see below)
Consider offering your patient guidance to fill out a caregiver affidavit if they are worried about child care while hospitalized or quarantined.
Consider including specific safety advice for people with substance use disorders.
Consider including a patient handout on public charge rules (English, Spanish, Portuguese) during COVID-19.
Consider including a patient handout on worker's rights during COVID-19.
ENCOURAGE PATIENTS TO CALL 211 OR 311, which will route them to the most up to date resources.
