Physicals
Physical examinations are required for all new students entering any grade. They are also required for students entering kindergarten , first , third and fifth grade. Please send in a copy of this physical to the nurse as soon as possible at the start of the school year. If I do not receive a physical from your child , he/she will be scheduled for an examination by the schools physician.
September 2019 -ATTENTION: Many students in kindergarten , first , third , fifth grade and new students to Merrimac are still missing physicals for this school year. Please make arrangements with your child doctor to have an examination as soon as possible ,and have the required physical form returned to me. If you have not handed in a physical from your private pediatrician your child will receive an exam from the school doctor.
