If accepted into the MACAP Program you will be required to set up an Immunization Tracking Package which includes the following:

- Proof of current Tdap vaccination
- Proof of Varicella vaccinations or positive titer
- Proof of Tuberculosis screening
- Proof of Hepatitis B vaccinations or positive titer
- Seasonal Influenza vaccination
- Proof of current Health Insurance
- Proof of current Physical Exam
- Signed Healthcare Provider Attestation Letter
- Signed Student Attestation Letter
