Girl Scout Facility Reservation Form - Complete guide on how to rent/reserve Girl Scout properties for Girl Scout members. Includes all forms needed.

GSNWGL Plan 2 Insurance Enrollment Form - Form required For trips to council owned properties or council sponsored events

GSNWGL Plan 3E Insurance Enrollment Form - Insurance form required for trips inside or outside of the Council involving overnight and day trips outside of Council

Health History Form - Each Girl Scout member should complete this form and return it to their troop leader prior to participating in any Girl Scout activities.

Money Earning Activity Application - Does your troop want to do a fundraiser outside of the Girl Scout Cookie Program and Fall Product Program? Complete this application!

Troop/Service Area Bank Account Information and ACH Authorization Form - Use this form to inform the Council about your Troop's banking institution and allow GSNWGL to deposit and withdraw money from your troop's bank account. All troop and service area are required to have a Troop/Service Area Bank Account Information and ACH Authorization Form on file with the Council.

Parent Permission Form - Use this form for special troop activities and return it promptly to your Troop Leader in order to ensure participation in the event.
