The National Mail Voter Registration Form can be used to register U.S. citizens to vote, to update registration information due to a change of name, make a change of address or to register with a political party.

Important: You must follow the state-specific instructions listed for your state. They begin on page 3 of the form and are listed alphabetically by state. After filling out this form, you must sign your name where indicated and send it to your state or local election office for processing. Be sure you mail it in an envelope with the proper amount of postage.

State Election Officials - Updating State Instructions on the National Mail Voter Registration Form:

Requests for updates to the state-specific instructions for this form must include the state election office's letterhead and must have an original signature of the appropriate representative/staff member in that office (digital signatures are not permissible). Request letters should be addressed to the Executive Director of the U.S. Election Assistance Commission.

Please do NOT use this form to register or re-register to vote if you are enrolled in a confidential address program such as Safe at Home. If sharing your address could put you in life-threatening danger, you may be eligible to register to vote confidentially.

If you are registering or re-registering less than 15 days before an election you will need to complete the Same-Day Voter Registration process and request your ballot in person at your county elections office or polling location.For more information on voter registration and voting locations, contact your local county elections official.

To register online you will needYour California driver license or California identification card number, The last four digits of your social security number and Your date of birth.Your information will be provided to the California Department of Motor Vehicles (DMV) to retrieve a copy of your DMV signature.

If you do not have a California driver license or California identification card, you can still use this form to apply to register to vote. However, you will need to take additional steps to complete your voter registration.

Visit your county board of voter registration and register in person. You can also register while receiving services at various agencies including the Department of Motor Vehicles. See below for a complete list of agencies providing voter registration under the National Voter Registration Act.

If your voter registration information has changed (name, address, etc.), and you still live within the same county, you may update your voter registration information using this form. If you have moved to a different county, you will need to re-register in your new county (see How to Register above).

Students may register to vote where they reside while attending college. This could be at an address in the college community (e.g., a dormitory) or at an address you intend to return to while not in the college community (e.g., family home). For guidance in determining what residence to claim for voter registration purposes, see S.C. Code of Laws Section 7-1-25.

If you are planning a voter registration drive at an institution of higher learning, please make sure you have the National Voter Registration Application for students who maintain their residency outside of South Carolina. Once a person who was convicted of a felony or offense against the election laws serves his sentence, he may register to vote. To register, the applicant must submit a new voter registration application to his county voter registration office. To participate in any particular election, the applicant must submit the application prior to the 30-day voter registration deadline for that election. In applying, the registrant is swearing under penalty of perjury that he is qualified to register, including having completed his entire sentence. County voter registration boards must be satisfied that the applicant has completed his sentence; and in some cases, may request a person who has lost his voting rights due to conviction to provide proof that he has completed his sentence.

Please note: After December 12, 2015, MCS-150 forms can ONLY be used to update a USDOT number, not for initial registration with FMCSA. First-time applicants must use the Unified Registration System (URS). For more information about URS, or to apply for the first time, click here.

These instructions assume that the applicant has determined that a USDOT Number is required to update the US DOT number registration and that the MCS-150 form must be submitted. Again, this form can only be used to update existing registrations as of December 12, 2015.

FMCSA strongly encourages applicants to use the electronic online update process since it has built-in edit checks and simple yes-and-no questions for easy, accurate completion. It is also significantly faster than applying by mail. The estimated time to complete the form online is approximately 20 minutes.

If the application is complete and you meet all qualifications to vote, the county board of elections will mail you a voter registration card. This mailing is non-forwardable and serves to verify your address. If the U.S. Postal Service returns a card as undeliverable, a second mailing will be sent. If the second mailing is returned as undeliverable, the voter registration will be denied.

If you submit your application form before the voter registration deadline for the next election, you should receive your voter registration card within two weeks. Contact your county board of elections if you do not receive your voter registration card.

You may be required to register to collect, accrue, and remit taxes or fees if you are engaged in activities associated with a tax or fee. If your business will sell taxable goods or services, you must register as a sales and use tax dealer to collect, report, and remit sales and use tax before you begin conducting business in Florida. To check the status of a submitted online registration or to retrieve your approved certificate number, log in to the Florida Business Tax Application using your user id and password you created for your online registration profile. Note: Allow three business days for the processing of a new application before checking the status.
