Participation on a Secondary School Athletic Team - Letter & Consent Form

All students who are on an athletics team need to have this form completed and returned. One signed form per sport. Student-athletes should already have a copy of this form from their coaches.

Athletic Fees & Uniform Policy Form 

The athletic fees are variable dependant on which athletic team one is participating in. This form includes a breakdown of all aspects of what the fees cover. Fees can be paid through SchoolCash Online and must be done within one week of the first game. Student-athletes should already have a copy of this form from their coaches.

Athlete Code of Conduct

Driver Authorization Form

For parents and other people who volunteer to drive students to and/or from games & practices, this form must be filled out and returned. Student-athletes should already have a copy of this form from their coaches.

Passenger List Form

For parents and other people who volunteer to drive students to and/or from games & practices, please inform coaches which athletes will be in which car.
