Civil Service Commission Resolution No. 2100020 dated January 7, 2021, prescribes the use of the revised application form for leave of absence, CS Form No. 6, Revised 2020, by all government officials and employees, a copy of which may be downloaded from the CSC website at www.csc.gov.ph.
Under the Local Government Code of 1991, the approving authority of the leave of absence of Provincial Governors, and Mayors of Highly Urbanized Cities and Independent Component Cities, is the Secretary of the Interior and Local Government.
All Provincial Governors, and Mayors of HUCs and ICCs, are directed to observe the following CSC guidelines in filing an application for leave of absence, for approval by the SILG.
In case of Vacation Leave, the application must be filed using CS Form No. 6, Revised 2020, five (5) days in advance, whenever possible, from the effective date of such leave. Vacation leave within the Philippines or abroad shall be indicated in the form for purposes of securing travel authorities, and completing clearances from money and work accountabilities.
Sick Leave shall be filed using CS Form No. 6, Revised 2020, immediately upon return from such leave. If filed in advance, or exceeding five (5) days from return, the leave application shall be accompanied by a Medical Certificate. In case medical consultation was not availed of, an affidavit should be executed by the applicant.
All applications received must be immediately forwarded to the Office of the Secretary thru channels at least four (4) days, whenever possible, prior to the start of the leave period, or in cases of sick leave, to be filed upon return, not later than four (4) days after such filing.
