Volunteer

In order to volunteer with the HSA each adult is required to submit:

1. PA State Criminal History Clearance (Act 34) 

2. PA State Child Abuse History Clearance (Act 151)

3. FBI fingerprinting  OR submit a Volunteer Affidavit  if you have lived in the 

state of PA for the last 10 years with no interruptions (Act 114)

When complete, send a PDF copy of your clearances to the HSA email (lewistownelem@gmail.com) or bring them to the Lewistown Elementary School Main Office.

Pennsylvania Criminal History Clearance (Act 34). To obtain this clearance, go to: https://epatch.pa.gov or click the link below. This clearance may be obtained at NO cost to you by selecting “New Volunteer Record Check” button.  In the "Volunteer Organization Name:" field, enter - "Lewistown Elementary HSA".

Pennsylvania Child Abuse History Clearance (Act 151). To obtain this clearance, go to:https://www.compass.state.pa.us/cwis/public/home or click the link below. This clearance may be obtained at NO cost to you by completing it as a volunteer.  First, "create individual account" unless you already have an account. 

Federal Criminal History and Fingerprinting (Act 114) To obtain this clearance, go to https://uenroll.identogo.com , When registering, please use Service Code: 1KG6ZJ . Click on Schedule or Manage Appointment.  The applicant will pay the current fee for the fingerprint service. - You ONLY need this clearance if you have not lived in the state of PA for the last 10 consecutive years.  As of Oct 6th 2023 the Mifflin County Library is an official IdentoGo fingerprinting location!

Volunteer Affidavit (Act 114) If you have lived in Pennsylvania for the last 10 consecutive years, you can complete the Volunteer Affidavit below instead of the Federal Criminal History and Fingerprinting Clearance as listed above.
