Legacy Preseason Camp Forms

Dates: February 9,10,11,12,13 2026

Where: Legacy Turf Fields

Time: 4:00 pm to 5:30 pm

Cost: $100 / Player, or $20/day of attendance

Players will receive 2 LHS Practice Shirts as part of the fee w/ 4 days of attendance.

Bring this completed form, with payment, to the Business Office during Business Office hours.  PLEASE NOTE: In order to do optional activities, your student account balance must be current.

• Appropriate Outdoor Soccer Attire

• Both Cleats and Running Shoes, Shin Guards and Water

In consideration of my application being accepted, I do hereby, waive and release, and forever discharge any and all rights  and claims for damages which I may have or which may hereafter accrue to me against Legacy High School, Adams School District #12 or their respective employees, officers, agents and/or representatives, for all damages which may be sustained or suffered by me in connection with my participation in the Legacy Girls Soccer Preseason Camp.  My dependent has permission to attend the camp and is in good, physical condition to handle the rigor of the activities involved.  I understand that I am responsible for any insurance while in attendance.  All proceeds go to Legacy High School Girls Soccer (Players equipment, uniforms, senior gifts, end of the banquet expenses, coaches gear and booster coaches salary).
