Guest Permission Form

If you plan to bring a guest who does NOT attend LCHS to the Homecoming dance, you must fill out the Guest Permission Form. A date or guest of an LCHS student attending a school dance must be at least a high school aged student and a Guest Permission Form must be completed and on-file in the LCHS Main Office.

Please be aware of the following guidelines:

Each Lakeview student is allowed one guest/date. Your guest must accompany you to the dance and depart with you.

Acceptance of guests is at the sole discretion of LCHS Administration.

Guests are required to comply with the Lakeview Dress Code. For more details, please refer to the Lakeview website.
