Scholars are required to complete this verification form once per year (generally May-June).

Thanks to our partnerships with Southwest, we have the opportunity to offer airline tickets to our scholars. 

Airline tickets are for educational purposes only and may be available for scholars studying outside the state of Colorado or for in-state scholars planning to attend out-of-state opportunities like internships, professional development, conferences, and more. 

Help us show our donors your community involvement by documenting any community service hours you complete during your time as a scholar. 

This community service form is not a requirement and can be completed at any time.

Scholars should complete this form approximately three months before their anticipated graduation date.
