• At least High School Graduate

• Can read, write, and speak basic English

• YP4SC Result

• 3 pcs 1x1 and 2x2 ID picture (colored, white background with collar blouse /shirt) 

• 1 photocopy (clear) birth certificate (NSO authenticated) / LCR

• Submit only one of the following :

  • • • (original copy) ALS card/result of exam

      • (original copy) High School Card (for High School graduate)

      • Transcript of Records & original Honorable Dismissal (for College level)

      • Photocopy and Original Copy of Transcript of Records (for College graduate)

• Medical Certificate

• Baranggay Clearance

• Certificate of Indigency from DSWD (for UAQTE Scholarship)
