My class always begins the year with a "Getting to Know You" form that helps me understand who they are. But I haven't modified it to emphasize inclusive practices.
The goal of this form is to redesign the Getting to Know You Form is to provide a foundation for goal development, highlight their "comfort" learning and help focus them on "reach goals", and provide a framework for them to understand how Universal Design for Learning will frame the course.
