Obtaining Medical Attention

·        For serious medical emergencies:

o   Call Campus Police Dispatch (202) 806-1100 (or 911).  Be prepared to provide the following information: a) name, b) location, and c) the type of injury.  Provide first aid and initiate life-saving measures, if necessary.  Stay with the injured person until the arrival of emergency responders.  Alert others in the building for assistance.  The injured person will be taken to the Howard University Hospital for treatment.

 

·        For non-emergency medical treatment:

o   Faculty and Staff will go to Faculty Practice Plan (FPP) which is located at 2041 Georgia Avenue (202) 865-7677.  http://huhealthcare.com/healthcare/doctors/contact-us

o   Students will go to Student Health Center (SHC) located at Medical Arts Building 2139 Georgia Avenue, 2nd Floor.  This is at the corner of Georgia Avenue and W Street NW.  Phone: (202) 806-7540, Fax: 202-806-7416, Phone lines open 8:30am - 5pm, closed for lunch 12:30pm-1:30pm. http://huhealthcare.com/healthcare/students

o   All guests on campus will go to Howard University Hospital (HUH) located at 2041 Georgia Ave NW, Washington, DC 20060.  The emergency contact number is (202) 865-6100.

 

Incident Report

·        All lab-related injuries must be reported within 24 hr using the Incident Report Form.  The form is available from Howard EHS https://ehs.howard.edu/index.php/resources/forms.  

·        Contact Department Chair and Dr. Steven Cummings to inform them of an incident.

·        If necessary, contact the appropriate University Emergency Response Team from Appendix A of the Contingency Plan for Hazardous Waste.  

 

Medical Waiver

In the event an injured person refuses treatment, the Medical Waiver form must be completed and signed.  The form is available at SHC, HUH, and FPP.
