Our care team

At Archuleta Integrated Healthcare (AIH), a care team takes an active role with you in your health and well-being. A variety of professionals may be part of your care team, depending on your health goals. This is a new approach to care at this location. A range of options is available to you, depending on your unique needs.

Your care options

  • Primary healthcare: We offer care across all life stages to anyone who needs primary care. Our care includes wellness visits, family healthcare, immunizations and chronic illness management.

  • Behavioral healthcare: We know that behavioral healthcare is an important part of your overall health. At AIH, we regularly check in with you to respond to any behavioral health needs you may have. We also offer a range of behavioral healthcare options, including counseling, substance use treatment, psychiatric evaluation and medication management. You can receive behavioral healthcare at AIH without changing your current primary care provider, giving you the options you need or want.

  • Oral healthcare referral: We offer referral options to the Axis Health System Oral Health Clinic.

Now accepting new patients

We welcome new patients, and offer a variety of payment options. Medicare, Medicaid, CHP+ and most insurances are accepted. A sliding-fee scale is available for those who qualify.

San Juan Basin Public Health serves Archuleta and La Plata counties with offices located in Pagosa Springs at 502 S 8th Street and Durango at 281 Sawyer Drive.

Services are provided Monday through Friday, 8 a.m. to 4:30 p.m., unless noted otherwise on program pages.

Human Services Resources

Department of Human Services (DHS)

The Archuleta County Department of Human Services provides financial, protection and prevention services to assist individuals and families. Assistance programs are provided to eligible residents in need and include: Supplemental Nutrition Assistance Program, Colorado Works, Health First Colorado/Medicaid, Child Care Assistance, Child Support, and Low-Income Energy Assistance, all designed to help families and individuals toward financial self-sufficiency. Protection & Prevention services are provided by child and adult protection services, along with community focused programming.

The Durango Community Shelter continues to be the only 24/7/356 shelter service in Southwest Colorado and the only option for families and individuals trying to get back on their feet and regain self-sufficiency. The facility has sleeping quarters for men, women, and families as well as communal living spaces including a kitchen, living room, laundry facilities, and dining room.

Temporary Stay Rules

Welcome. We are here to give you a hand-up rather than a hand-out. The hand-out will be for a very short time, a day or two. The hand-up is designed to help you get on our feet. We aren't a homeless shelter for unlimited stays.

What We Offer

We are providing this temporary space for you to seek what the Almighty Loving God wants you in this life. To accomplish this for you and for the others the following rules are needed...

What We Expect

Need a plan and to work it. If you don't have one, you need to make one immediately. Else you fall into the hand-out stay. Must Attend church and bible Study. No drinking or smoking pot or other drugs or coming home under the influence. Must get along with others, no fighting or bickering. Clean up after yourself and others. No smoking inside. Must be engaged in work or looking for work. No visitors after 9pm. Quiet time after 10 pm and must be back onsite by this time.

Domestic Violence

Rise Above Violence (Rise) is a private nonprofit organization. Our primary mission is to assist victims of Domestic Violence and Sexual Assault. We work to ensure victims rights are exercised and needs met.

Through the support of grants, donations, fund-raising efforts and volunteers Rise provides the following services:

  • Immediate crisis intervention available 24 hours a day by an advocate trained to listen and provide support. Call anytime day or night at (970) 264-9075.
  • Court Advocate to explain the criminal justice system and assist with civil restraining orders.
  • Emergency transportation to safe shelter. Shelter provided by the SW Safehouse.
  • Advocates to assist with other community agencies and supportive organizations.
  • Information and referrals.
  • Speakers and trainers to provide community education.

We recognize that domestic violence is a repeating pattern and that recovery from either domestic violence or sexual assault can be a lengthy process. While our advocates are not professional counselors, they do understand. They are trained to assess your needs identify and discuss your options, access available services and provide information and referrals to other agencies available to help with your long-term issues. Rise also has a court advocate on staff to explain and assist with the legal process, including help with filing of restraining orders.

WIC

WIC is the supplemental Nutrition Program for Women, Infants, and Children — a federally-funded program that provides free healthy foods, nutrition education, breastfeeding support, and referrals. WIC serves women who are pregnant, breastfeeding, and/or up to 6 months postpartum, infants, and children up to 5 years of age.

Addiction Support

SAMHSA’s National Helpline, 1-800-662-HELP (4357),(also known as the Treatment Referral Routing Service) is a confidential, free, 24-hour-a-day, 365-day-a-year, information service, in English and Spanish, for individuals and family members facing mental and/or substance use disorders. This service provides referrals to local treatment facilities, support groups, and community-based organizations. Callers can also order free publications and other information.

What are the hours of operation?

The service is open 24/7, 365 days a year.

How many calls do you receive?

The Helpline received 573,946 calls in 2015. That represents nearly 47,826 calls per month. In 2016, the Helpline call volume has increased to an average of over 65,000 per month.

Do I need health insurance to receive this service?

The referral service is free of charge. If you have no insurance or are underinsured, we will refer you to your state office, which is responsible for state-funded treatment programs. In addition, we can often refer you to facilities that charge on a sliding fee scale or accept Medicare or Medicaid. If you have health insurance, you are encouraged to contact your insurer for a list of participating health care providers and facilities.

Will my information be kept confidential?

The service is confidential. We will not ask you for any personal information. We may ask for your zip code or other pertinent geographic information in order to track calls being routed to other offices or to accurately identify the local resources appropriate to your needs.

Do you provide counseling?

No, we do not provide counseling. Trained information specialists answer calls, transfer callers to state services or other appropriate intake centers in their states, and connect them with local assistance and support.
