Students will be assessed on their personal traits and sports skills in the forms provided.

You may access the class assessment forms via the Google Sheet. For instructions on how to access the assessment forms, kindly view the video.
For Personal Traits, please click the box if you observed the student display the trait during the course of JSA.
For Skills Assessment, please fill in 1 to 4 for each skill according to the Skills Assessment Rubrics.
