What does my child need in order to play sports at JP Case?

Your child needs a current physical examination on the New Jersey State physical forms filled out in its entirety. This includes but is not limited to; the entire 'History Form' completed including all "Yes" answers explained, the entire 'Physical Examination' page completed including an eye exam with a passing visual acuity score done by your physician or eye doctor, and the entire 'Clearance Form' completed with a signature and stamp from your physician and their office.


How can I turn in my forms?

We will not be accepting physical forms sent electronically, please hand in a hard copy of all necessary forms to the main office during the summer months. Please keep a copy for your own records.


What if my child's physical falls after the cut off date?

If your child's physical appointment falls after the August 3rd cut off date and it cannot be moved to an earlier date don't sweat! Here's what you need to do, register on Family ID, hand in the physical they currently have as well as the Health History Questionnaire. Communicate to us when your child's physical will be so that we know they are interested in playing a sport and we can follow up with you!


My child played sports for JP Case last year, do I need to hand in anything?

YES! Your child needs current documents to play sports at JP Case! The Health History Questionnaire must be completed for each season and a physical is only valid for 365 days. 


My child already took the ImPACT test, do they need to do it again?

NO! The ImPACT test is valid for 2 years, student athletes will only need to take 1 baseline exam during their time at JP Case.
