Athletic Registration 

REGISTRATION DATES - All requested paperwork needs to be submitted to the nurse by the date registration closes to ensure the School Physician can review all documents to clear the student-athlete. 

Fall Registration Opens: June 11, 2024

Fall Registration Closes:  August 8, 2024

Physicals must be valid from 9/9/23-9/9/24

First day of tryouts: September 9, 2024

Winter Registration Closes: October 20th

Physicals must be valid from 11/13/22-11/13/23

First day of tryouts: November 13th

Spring Registration Opens: December 4th

Spring Registration Closes: February 2, 2024

Physicals must be valid from 3/18/23-3/18/24

First day of tryouts: March 18th

What does my child need to participate in a sport at J.P. Case?

• Register on FamilyID

Print the following forms and submit a hard copy to the J.P. Case Health office 

• Physical forms  (En Español) and Health History Update Questionnaire  (En Español) need to be completed and signed by a physician.  Physicals are valid for 365 days prior to the first day of tryouts. Health History Update Questionnaire needs to be turned in for each season.

• Have your child take the ImPACT Test at home.  Only done once during your time at J.P. Case.

• If your child has asthma and/or allergies requiring an epi-pen, you must submit a current Asthma Treatment Plan and/or an Allergy Action Plan with your physical forms.
