Athletic Registration 

Please be sure to complete the following steps to register your child for J.P. Case sports!!

Step 1: Register Online 

• Register your child on FAMILY ID

• This should be done for each sports season they try out for.

Step 2: Submit Health Forms to the School Nurse

Required Forms:

• Physical Form (En Español available)

  • Must be completed and signed by a physician.

  • Valid for 365 days before the first day of tryouts.

• Health History Update Questionnaire (En Español available)

  • Required for each sports season your child participates in.

Important Note:

• Only submit the medical eligibility form  (from your doctor) and the Health History Update Questionnaire (En Español available) to the nurse.

• Do NOT submit a copy of the entire physical form.

Additional Forms (If you have asthma or an allergy requiring an EPIPen):

• Asthma Treatment Plan – if your child has asthma.

• Allergy Action Plan – if your child requires an EpiPen.

  • Athletes must be able to self-administer medication.

Step 3: Take the ImPACT Test

• Your child must take the ImPACT Concussion Baseline Test at home.

• This is a one-time requirement during their sports career at J.P. Case.

All requested paperwork must be submitted to the nurse by the date registration closes to ensure the School Physician can review all documents to clear the student-athlete.
